Dental Implants

Dental implantology is a modern procedure for the replacement of lost and missing teeth. A dental implant means a solution for many problems posed by removable prothesis. While several people are satisfied with their traditionally made removable dentures, according to surveys for 85% of prosthesis wearers it still causes problems, complaints and means solutional disadvantages in their everyday life.

A removable dental prosthesis can badly affect the timespan of remained natural teeth. With the help of dental implantology these problems and troubles can be ceased. In case of incomplete loss of teeth the use of removable partial dentures becomes unnecessary since implant-supported fixed bridges are possible to be made instead.

People who chose to have dental implants reported that the quality of their life largely improved as well as their self-esteem was re-established due to the fact that they didn’t have to use removable dental prostheses any more. At the same time implant-supported teeth play an important role in both keeping the remaining natural teeth safe and in the conservation of the health of the mouth and teeth.

What is necessary to know about dental implants?

Dental implants are part of an implantation system based on “bone integration” which is scientifically justified and widely spread around the world. The professional term of „bone integration” means that the implant, made of “tissue-friendly” substance and in an appropiate form, is placed in a suitable case in the upper or lower jaw with the right technique of operation so that it gets into a direct connection with the bone and fuses with it.

A dental implant functions as a model of structure similar to the construction of a natural tooth. They can be applied in a great variety of cases and they can also be combined with existing teeth or even with each other.

At all times implant can be applied according to individual demands and requirements. With an implant-supported replacement chewing capacity is comparable to that of natural teeth.

Who is a suitable person to have dental implants?

Generally anybody who is healthy enough to have a tooth extracted. Certain diseases and states of health reduce the simespan of dental implants and maybe exclude the possibility of receiving dental implants. For that reason careful medical, dental examinations as well as X-rays are carried out before all dental treatments like that.

Mostly the main limitation is the quantity of and quality of the bone available which can be evaluated by a special X-ray. Dental implants can provide a lasting solution for the dental problems of those who care for their teeth properly and regularly.

What kind of replacement opportunities are provided by dental implant?

  1. Replacement of one missing tooth: the implant and the crown seated on it are suitable for replacing the only missing tooth without the necessity of grinding the teeth adjacent to that.
  2. Removable partial denture is unnecessary since implants in themselves or being combined with existing teeth also make the application of fixed bridges possible.
  3. Stabilizing dental prostheses: Implants are also available for fixing removable full dentures more effectively. It means a great benefit especially in case of lower full dentures.
  4. Fixed replacement in case of no teeth left: By using at least six implants a fixed replacement, so called “fog-bridge” can be made.

How does the placement of the implant happen?

The procedure of placement itself is performed in the form of oral surgery operation on a one time occasion in the dental office. The operation takes place in local anaesthesia and the implant is placed into the bone under the gum.

After that comes the period of healing reaching as far as 3-6 months during which the integration of bone is completed, i.e. the implant gets fixed with the bone. Following the healing period a second short operation takes places when the mycoderm above the implant is opened and part of the implant serving to fix the bridge gets installed finally. Later we make a model copy the way as usual in order to construct the replacement teeth in the lab.

What nuisances does the implantation have?

According to most patients who have already gone through the procedure the placement of implants means nuisances similar to that of a tooth extraction. In case the patient has worn a denture before, he can use it again a couple of days after the operation when we make it suitable for that. So he doesn’t have to change the usual way, he lives during the 3-6 month period of healing either.

How long does an implant last?

Based on researches when the so-called „bone integration” takes place, the implant performs its task in the bone for quite a long time. It can happen that the implant must be removed in spite of the thorough examinations, careful planning and operation. (Because of stiffness, inflammation, allergy or pain.)

If the implant doesn’t get fixed adequately or it is in the healing period only, it can be usually removed easily.

Supposing implant is fixed partly or totally it can be removed with a special drill during a small surgical procedure in local anaesthesia.

After the removal a hole similar to that of a tooth extraction can be found which heals similar way. In these cases thereis an opportunity to carry out implantation again generally.

The succes of the procedure of implant placement depends on several factors: the patient’s general state of health, proper oral hygiene, anatomic variations, and the technical implementation of replacement.

In the long-term maintenance of dental implants proper oral hygiene, as well as exceptional care and cleaning are considered to be determining.

Implant-supported replacement cannot be regarded as something separate from the chewing apparatus as a whole because it is to provide the health and the right function of the complete chewing organ.

For that reason regular dental check-ups are highly advised even if there are no complaints.

The process of implantation:

  1. Definition of status, laboratory and X-ray exams. Planning.
  2. Operation.
  3. Removal of sutures in 7-10 days.
  4. Bone integration (waiting period of 3-6 months).
  5. X-ray controll exam, the part of the implant which serves to fix bridge gets installed. Making a model (copy) of teeth. The construction of the replacement teeth (about 1 week).
  6. Replacement.

CAD/CAM

CAD-CAM dental technology for high-precision dentures

In addition to esthetic expectations, another important requirement, durability, is achieved through a precise fit.

Traditional casting technology is replaced by CAD-CAM technology, with computer-controlled milling technology replacing manual milling. The result is a perfect, homogeneous crown that fits with nanometer precision.

The structure of CAD-CAM technology

CAD (Computer Aided Design) is the software system of CAD-CAM, i.e. computer-aided design. With the help of the program, the plaster model, which has been scanned by sensors, appears on the monitor and a digital pattern is created on which the abutment can be visually planned. The dental technician can adapt the framework of the crowns to this digital pattern, thus ensuring a perfect fit of the restoration.

CAM (Computer Aided Manufacturing) is the “executive” part of the CAD-CAM system. This framework of the crown, produced with the milling machine, is always of the same quality and consists of homogeneous, oxidation-free cobalt-chrome, titanium or zirconium. The CAM unit is a computer-controlled milling machine. This homogeneous framework is covered by the dental technician with a compatible ceramic, which is adapted to the esthetic, biomechanical and hygienic needs of the patient.

Aesthetic high-tech crowns based on zirconium oxide

As a modern dental practice, we can meet the wishes and requirements of our patients to have an aesthetically pleasing white smile. The high aesthetic expectations are a great challenge for dentists and dental technicians.

What is zirconium oxide?

Zirconium oxide is the oxide of the zirconium element, combined with oxigen. It is a very hard, strong material that can also cut steel. Zirconium oxide has very high resistance, high flexural strength and hardness. To achieve these properties, zirconium oxide is treated in the laboratory using a special process. Due to the production technique, the bending strength of the porcelain is approximately ten times that of normal porcelain. Due to the great hardness of the material, it was not used in dental technology for a long time, as it could not be processed using the usual (traditional) processing techniques (casting, firing or pressing).

In today’s modern dentistry, the rapid development of computer technology has led to the development of CAD-CAM technology. This has made it possible to process and use zirconium oxide.

Today, dental prostheses are customized by CAD-CAM systems according to precise planning.

The advantages of zirconia crowns are numerous, not only from a health but also from an esthetic point of view:

1. As a solution for metal allergies

In the past, it was impossible to provide dentures for patients with metal allergies because the allergic reactions caused more problems for the patient than the missing denture itself. This problem was completely solved by zirconium crowns. It is true that all-ceramic dentures were available before, but the material was not strong enough and not a long-term solution.

2. Natural appearance due to high translucency

The natural tooth is translucent up to a certain point and it was not possible to achieve this appearance with the existing technology. With the development of the zirconia crown, this problem was solved due to the color identity and transparency. The appearance of the porcelain veneered zirconia crown is identical to the denture, so the denture looks naturally real.

3. No more gray-black tooth neck edge – as used to be the case with metal-ceramic crowns

In the past, many people found it aesthetically unpleasant that the receding gums caused by gingivitis made the gray edge of the metal framework stand out. It was a natural reaction of the body against metal. Zirconium crowns are biocompatible and therefore this reaction does not occur. The prepared tooth protects the periodontium and the dreaded inflammation and receding gums are a thing of the past.

4. Higher resistance

A major disadvantage of conventional ceramic dentures was that the framework was not strong enough. For this reason, it was not the ideal material for pontics. The high strength of today’s zirconia crowns has eliminated these problems.

5. Individual fit and comfort

With conventional metal framework restorations, users were always forced to make compromises.

One of the disadvantages was that the patient felt a metallic taste in the mouth. As the zirconia crown is completely metal-free and therefore not electrically conductive, there is no metallic taste in the mouth.

The thermal conductivity, which also leads to heat sensitivity with metal crowns, is therefore also eliminated. There is no increased heat conduction with zirconium oxide crowns.

6. Usability with implants

With its high strength, crowns and pontics can also be used for implants.

7. Accuracy and precision

Special computer technology is required for the planning and realization of zirconium oxide crowns.

This precision ensures a perfect fit, which makes wearing a zirconium oxide crown much more comfortable compared to other dental restorations.

The many advantages of zirconia crowns are offset by a single disadvantage. Zirconium oxide crowns are more expensive than conventional dentures. The quality of zirconium oxide crowns due to longer durability and compatibility justify the higher investment costs. This also results in a better quality of life and an effect similar to that of your own teeth.

In modern dentistry, lithium disilicate is mainly used for inlays and onlays.

Lithium disilicate is a glass ceramic, is biocompatible, highly stable and extremely durable. Due to its compatibility, it is very gentle on the opposing teeth. The glass-ceramic owes its high aesthetics to its translucency, and a perfect fit in the mouth is also guaranteed.

DISCRETE

A frequently asked question is whether the teeth or gums will be damaged. If it is done by a dentist, it is not. The whole thing takes a maximum of fifteen minutes, and then it’s “smile and shine”. If you no longer want to wear it, you can remove it without any problems. Similar to tartar, the tooth is polished afterwards and treated with fluoride gel.

Jewelry can also have risks and side effects

Experts advise against grinding or drilling into teeth to attach jewelry. This damages the tooth’s natural protective coating – the enamel. Germs can then penetrate the tooth more easily and cause tooth decay.

Careful oral hygiene, including the use of dental floss and antibacterial toothpaste, is important, as food residues and plaque can easily build up on the dental jewelry, which can lead to the development of caries around the bonding points, but also to inflammation of the gum line. Any form of dental jewelry can lead to abrasion of the adjacent teeth.

Metal caps almost always damage the gums and teeth, both when inserting, wearing and removing them. The cap irritates the gums and germs can settle in the crevices between the metal, the gum line and the tooth. This can lead to periodontitis and tooth decay. However, there is always a risk of microbes collecting in the spaces between the tooth and jewelry. This is why dentists generally advise against these caps.

Gilding

Problem: incompatibility of certain metals

More and more patients are showing allergic reactions to metals – mainly to those that do not contain precious metals, such as those used in the manufacture of partial dentures.
In order to provide these people with suitable and compatible dentures, we can either use an acetal model casting or electroplated gold plating of a conventional non-precious metal model casting.
This involves chemically applying a wafer-thin layer of fine gold.
This is absolutely firmly bonded to the model casting and prevents any contact between the gingiva and the non-precious metal, despite the thin layer.
Subsequent gilding of older work is also no problem for us.

Aesthetic perception

The thought of removable dentures makes many people shudder.
Especially with clasp dentures, there is the uncomfortable feeling of no longer having your own teeth in your mouth, the constant visibility of the sometimes very dominant clasps.
Although we cannot make them disappear with this type of restoration, we can make them appear more valuable by gold-plating them.
The gold reflects the light in a much warmer shade than the silvery non-precious metal (NPM), which often appears black in the mouth.
For most patients, this casts a better light on their model casting prosthesis.

Root canal treatment with machine

Tooth decay develops in the teeth, causing bacteria to enter the root canal, which leads to toothache.

If we visit the dentist at the first symptoms, root canal treatment or tooth extraction can usually be avoided.

Without medical treatment, the inflammation will lead to necrosis of the tooth. The easiest way is to extract the painful or dead tooth, but the gap remains.

The removal of the nerve is completely painless with proper anesthesia. After the root canal treatment, no anesthesia is required for further steps. If the treatment has been carried out with great care, no more pain is to be expected.

Until the inflammation has subsided, there may still be increased sensitivity when chewing.

Root canal treatments are up to 95% successful. The main causes of errors are inaccurate measurements or branches of the root canal that are difficult or impossible to clean. These additional root canals are not even visible on the x-ray.

An important step in root canal treatment is to completely fill the canal so that there is no room for bacteria. To ensure that the root canal filling is exactly as long as the root canal itself, its length must be precisely determined.

The length of the root canal used to be determined with the help of X-rays, but today digital length measurements based on an electrogalvanic method (apex locator) are available.

This method is much more accurate than x-rays, so successful root canal treatments are greatly increased. In addition, these digital methods have no harmful effects on health, unlike X-rays.

Root canal treatment with a machine has opened up new horizons and possibilities in the “cleaning” and “processing” of the root canal.

The treatment is better and more precise thanks to the machine.

Advantages of root canal treatment with the machine:

• faster, more precise cleaning and treatment
• short treatment times for the patient
• effective tooth preservation through root filling
• can also be used for curved canals.

What can be expected after root canal treatment?

The root canal is no longer alive, which means that not only the nerves but also the blood vessels have been removed during the treatment. This prevents the organic substance and moisture supply to the teeth.

The color of the tooth changes (becomes grayer), as does the structure (can break more easily), but the tooth can still provide “good service” for a long time, either independently or as a fixation for a dental prosthesis.

You can receive a detailed plan of dental treatment and a price offer via e-mail, by fax or by mail. You only have to send a panoramic X-ray or the treatment plan of your own general dentist. The expenses differ according to the type of dental treatment, it extention and the substances used int he procedure.

The treatment plan is made according to personal needs. The following data are only informative:

  • To make a full denture 2 treatments are necessary with 2 or 3 days in-between them.
  • To make crown, bridge or inlay 2 treatments are necessary with a week in-between them.

Of course, we make receipts about each dental treatment.

At the beginning of treatment you have to pay a deposit and the remaining part of the amount is to be paid after the dental work is done (you will get a detailed receipt about that).

Health Insurance Funds abroad accept our receipts but we recommend our patients that they inquire at their own Health Insurance Fund in advance and get the necessary documents which we are ready to fill out.
